What Is The Chronic Disease Home Management (CDHM™) Programme For Diabetes?

Speedoc’s Chronic Disease Home Management (CDHM™) is a fully home-based treatment programme designed to help you manage your chronic conditions optimally, right from the comfort of your home. All our CDHM™ programmes are strictly guided by the Ministry of Health’s clinical practice guidelines. Chronic conditions we treat under CDHM™ include:

  • Diabetes

  • Hypertension (a.k.a high blood pressure)

  • High Cholesterol (a.k.a hyperlipidemia)

  • Thyroid Conditions (hypothyroidism and hyperthyroidism)

This programme is fully customisable to your medical history and needs. Took a blood glucose test just a month ago and don’t want to take it again? Let us know and we’ll remove it from the list of tests you’ll receive as long as you still have the results with you. Need an additional blood test? Add it on to the list. Our Patient Care Team will listen to your needs and help you craft your very own CDHM™ programme.

WHAT IS INCLUDED IN CDHM™ FOR DIABETES?

Our CDHM™ programme for diabetes includes:

  • All relevant lab tests

  • One (1) doctor house visit accompanied by a nurse

  • One (1) nurse house visit

  • Three (3) video consultations with a doctor to review your condition

You will also receive five (5) additional video consultations that can be used by any family member, for any condition, within the 12 months you’re enrolled in the programme.

HOW OFTEN WILL I SEE THE DOCTOR UNDER THIS PROGRAMME?

You will have a total of five (5) consultations with your doctor:

Consultation 1: Initial visit (in-person) Your doctor will visit you at home with a nurse to conduct the necessary tests and examinations to set a baseline for your treatment for the upcoming year.

Consultation 2: Follow-up teleconsultation (3-5 days after initial visit) Approximately three to five days after your first visit, your doctor will schedule a follow-up video consultation with you to go over your test results and clarify any doubts you may have. Three months’ worth of medication will also be prescribed and delivered to you, free of charge.

Consultation 3: First review over teleconsultation (3 months after initial visit) Your first review will be done three months after your initial consultation. Our Patient Care Team will contact you ahead of time to schedule your review. This consultation will be done over video consultation, where your doctor will review your condition and make the necessary adjustments to your medication if needed. Another three months’ worth of medication will be prescribed and delivered to your home.

Consultation 4: Second review over teleconsultation (6 months after initial visit) Prior to your second review 3 months later, a nurse will visit your home to take blood samples and perform a diabetic foot screening. He/she will also provide lifestyle and diet counselling. Your second review with your doctor will be scheduled about five days later, when your lab results are out. This consultation will also be done over video consultation. Your doctor will explain your test results to you and prescribe another three months’ worth of medication accordingly.

Consultation 5: Third review over teleconsultation (9 months after initial visit) You will have your final review of the year enrolled in the CDHM™ programme 9 months after your first consultation. You will meet with your doctor over video consultation again to review your condition and have your final prescriptions done. After this consultation, you will have the option of renewing your enrolment.

WHO SHOULD SIGN UP FOR THE CDHM™ PROGRAMME FOR DIABETES?

Anyone who suffers from diabetes would benefit from enrolling in CDHM™ for diabetes. In particular, this programme would be suitable for those with type 2 diabetes that is not well-managed as we remove the hassle of remembering to go for follow-ups.
